Support24 is specially designed to help people live independently in the comfort and security of their own home it is a flexible service which can respond quickly to any changes in your circumstances Christine and Linden will take us to a typical day in the life of a support worker. I am going to meet a new customer today. We will spend quite a bit of time talking about the support she needs and we'll agree a support plan so that she know's when I'll be visiting and what help I can give her. Every customer has a support plan which is unique to them the support worker will review the plan with you on a regular basis I am visiting a gentleman who's recently had a Telecare Alarm fitted he's still pretty active but he wanted it for his peace of mind I'm going to check that he's happy with it and that he understands how it works. Telecare Alarms are suitable for customers of all ages and abilities, they are easily installed and linked to a control centre which is open 24 hours a day 7 days a week I am going to see a lady I visit once a week and we use the time to talk about how her week has been and I help her to deal with any paper work such as filling in forms or dealing with bills so instead of worrying on her own, she knows that she can save them for me and we'll sort them out together The support worker can offer a wide range of services including helping you stay well active and healthy spending time listening to any worries or problems you want to talk about Support workers will always take time listen whether it's to catch up on the latest family news or just chat through a worry we are happy to be there sometimes by getting to know about a customers hobbies or interests we can tell them about local events or activities that they might like to join We all know that staying fit and active is so important but not everyone is able to go to a gym I am going to an Extend exercise class to see if it would help a gentleman I visit who wants to be more mobile Extend exercise classes are open to anyone who feels they want to improve their fitness or become more mobile Extend is gentle exercise to music in a relaxed and friendly environment the exercises all specially designed to help flexibility, improve balance and increase coordination which can help to prevent falls. This lady is recovering from an eye operation feels a bit nervous so she's asked for some more visits I'm going to go through her support plan to agree new days and times and also to see if there's anything else that we could do to help Support plans are designed to be flexible and responsive if circumstances change or you need extra support after an operation or stay in hospital support visits can be easily and quickly increased as needed if you have a lot of tablets or medicines to take it can be very confusing and missing or mixing up a dose can seriously affect your health although support workers cannot give medicine to customers they will work with chemists and arrange for daily doses to be put into special containers this makes remebering to take medications so much easier I think one of the best parts of my job is knowing that I can help people to stay in their own home for as long as they want even if their health changes By helping this lady to apply for some adaptions such as the shower and some grab rails, she can continue to enjoy her independence for many years to come At the end of the day the support workers make sure that details of their visits are carefully and safely recorded they contact other agencies on customers behalf and arrange visits or appointments if required Support24 meets all of the requirements of the Supporting People Program Achieve economic well-being Enjoy and achieve Stay healthy Stay safe Make a positive contribution
