In order to receive accommodations through
Disability Services you must provide
documentation of disability.
Documentation should include diagnosis of disability
from a professional.
Examples of documentation include a
psycho-educational or psychological assessment,
letters from physicians, psychologists or psychiatrists,
CNIB card or audiology report.
If you have an IEP or Individualized Education Plan
from your high school, look it over or contact your
high school to see if a psycho-educational or
psychological assessment was completed.
If so, provide this to Disability Services.
To obtain a copy of your assessment,
you can contact your high school’s guidance or
special education office or your school board.
Documentation is held in confidence by our office
and not released to college faculty.
Diagnosis of disability is only released with the
express written consent of the student.
If you suspect you may have a learning disability,
but have not been formally diagnosed,
you can meet with a disability consultant who
may be able to help guide you in getting a
psycho-educational assessment completed if necessary.
