Term Applying For: Mark the box of the semester you are applying for and write in the year.
Full Legal Name: Write in your last name, first name and middle name if you have one.
Previous Name on Academic Records: You can go ahead and leave the section blank.
Personal Information: Please write in your Social Security number, date of birth and an e-mail
address where we can contact you if there's an issue with your application.
Mailing Address: Write in your street address, city, state and zip code.
Also write in your telephone number.
Permanent Address: If it's the same as the mailing address that you just wrote above just
mark the box. If it is different, please write in your new address.
Citizenship: If you're a U.S. citizen which means you were born in this country or you became a citizen through the naturalization process
marked box number 1.
If your Permanent Resident, mark box number 2.
If you're a Temporary Resident, mark box number 3...
and if you're Refugee or an Asylee, mark box number 4.
If you marked box 2, 3 or 4 you will notice below that it states: "If 2, 3, or 4 the following is required...
